An established NDRI with surprising nicotinic antagonism and a quietly weird pharmacology

Bupropion is one of those drugs where the official label says "NDRI" and that's true but it undersells how strange the molecule actually is. It's an aminoketone, structurally related to cathinones, and the pharmacology is a mix of dopamine and norepinephrine reuptake inhibition, nicotinic acetylcholine receptor antagonism (particularly at alpha4beta2 and alpha3beta2 subtypes), and active metabolites (hydroxybupropion and threohydrobupropion) that contribute significantly to the effect profile and have their own receptor pharmacology.

The reuptake inhibition is actually pretty modest in absolute potency terms (Ki values in the low micromolar range), so people have argued for years that the clinical effects probably depend as much on the nicotinic antagonism as on the monoamine reuptake piece. The smoking cessation efficacy (Zyban indication) supports this, since the nicotinic antagonism gives you a coherent mechanism for nicotine craving reduction thats independent of mood effects.

What we like: this is one of the cleaner activating antidepressants in the sense that it doesnt cause sexual side effects like SSRIs do (because no serotonergic activity to speak of) and tends to be weight-neutral or weight-reducing rather than weight-promoting. The nicotinic antagonism angle is underappreciated. And the long clinical track record means the safety profile is well-characterized including the seizure threshold reduction (genuine concern at higher doses), CYP2D6 inhibition (drug interaction issue), and rare hypertensive responses.

What we dont like: the multi-mechanism profile makes it hard to use as a clean research tool for any single receptor system. If you want pure NET inhibition, atomoxetine or reboxetine are cleaner. If you want DAT inhibition, methylphenidate is more selective. Bupropion is a polypharmacology compound. Useful clinically, awkward as a probe.

Mechanism: partly mapped. The active metabolites complicate the structure-activity story because the threo enantiomer of the hydroxy metabolite has different selectivity than the parent.

Sourcing: well-established generic pharmaceutical, racemic mixture in standard formulations, research-grade is widely available, individual enantiomers are commercially obtainable but more expensive. Quality is generally fine from reputable vendors.

Abstract

Bupropion (Wellbutrin, Zyban; CAS 34911-55-2; molecular formula C13H18ClNO; molecular weight 239.74) is a unicyclic aminoketone developed by Burroughs Wellcome and approved by the FDA in 1985 for major depressive disorder, with subsequent approval for smoking cessation (1997) and seasonal affective disorder. Mechanism is selective inhibition of norepinephrine and dopamine reuptake (NDRI) plus non-competitive nicotinic acetylcholine receptor antagonism. The active metabolite hydroxybupropion contributes substantially to the antidepressant activity. Pharmacokinetics: parent half-life ~14 hours; CYP2B6 metabolism. Reported research dose ranges in the literature are described in the source publications. Lowers seizure threshold; contraindicated in seizure disorders and bulimia.
